When you submit your American passport application, in addition to Form DS-11, you will need to present several documents to prove citizenry. You may also have to submit other forms that cover special cases. Use this form if it has been more than 15 years since you received your original document. It is also for those who have never received one and this is a new passport.

Obtain Form DS-11 online or at a local facility that processes them for your replacement US passport. This document cannot be submitted through the mail because you must sign it in front of an official who must confirm your identity. They will ask for identification to verify you are the person submitting the application.

A passport agency or acceptance facility can accept your application and witness signatures. The facilities include county clerks, post offices and libraries. Check in your area to confirm where the nearest facility is located. Agencies include the field offices that are allowed to accept, witness and process applications. They are in designated cities with high populations.

Expediting companies are licensed to accept and process your application as well as to hand-deliver it to the local Agency for same-day processing. The number of applications that can be processed in this way is limited so it may take longer than one day to complete. There is a cost associated with expediting companies. Make sure to confirm the cost, since same-day service can be quite expensive.

When you submit your documents, you must provide primary evidence of U. S. citizenship. The options that you can choose from include a previously issued passport, a certified birth certificate or a Certificate of Citizenship. Check online for the full list of documents that can be submitted to prove you are a U. S. Citizen. When you receive your completed travel document, your documentation will be included with it or may be mailed under separate cover.

Present identification is required to prove your identity when signing documents. There are several options that will satisfy this need, including an expired passport, a valid driver’s license or military identification card. You will not be required to surrender a present identification information but do need to provide a photocopy of both sides. It must be on plain white paper.

For those who submit an out-of-state primary identification, a secondary form of identification must also be supplied. The additional documentation must include your name, photo, date of birth and issuance date. Once again, the secondary identification must be copied on standard paper and both sides must be copied.

Appropriate identification must be supplied for the person submitting an application for a disabled person who cannot go to the office to sign. The same rules apply including a photocopy of each side of their identification document. Identification must also be provided for a witness who identifies the disabled person’s signature.

Include a two-inch by two-inch photograph of the applicant, in color, when submitting the passport application. Since it will be used to create the replacement United States passport, the photograph should not be attached in any way. There are several specific requirements for the photograph and they all need to be followed. Check to make sure yours meets the rules. When all of the documentation has been signed and submitted, the fees associated with the travel document must be paid.
