What you will need to get a USA Passport application processed requires some work on your part. A few identifying items have to be supplied along with the form to apply. The Department of State Form DS-11 is the application that will be filled out. It is available online. The form can be filled out on the computer and printed or it can be printed and hand written. It is important that you do not sign this paper until you are in front of an agent. They must witness your signature.

Generally proving citizenship is merely bringing your birth certificate with you. However, years ago it was common to have a proof of birth certificate given by the hospital. This often was thought to be a certified certificate. A certified copy from the state’s registrar is needed. It must be signed by the registrar and be dated within a year of birth. The seal too is needed as proof of authenticity. Alternatively, an old passport that is legible also can be used.

Copies of the IDs must be supplied with the form. Both front and back must be shown on a standard-sized white piece of paper. Copy is to appear only on one side of the paper, no additional information should be on the paper. The copied image can be larger but not smaller than the original identification piece.

The payment of fees will be expected when you apply. Accepted payments can come from credit or debit cards, personal or bank checks or money orders or exact cash. These will vary by office, as will how the fees are charged. The two fees may need to be paid for separately. Call the agency before your appointment to understand their policies.

Two two-by-two inch colored photos must also be submitted. There are local establishments that can provide the photography that meets the government requirements. The photos must be identical, taken within the last six months, and be a true representation of the person’s likeness.

The process of obtaining a US passport application is not difficult. Take the time to make sure you have all of the correct information and documents. The entire process and the form can be found online. Expect about a four to six week processing period even for a replacement passport.
